In addition, be sure to include:
  • your GUP number
  • the Principal Investigator, or GUP Spokesperson.

on the exterior of your package.

Shipping Samples Spreadsheet

Identifying the contents of your return shipment is crucial for the timely return of your items.  You can expedite handling of your package by the SRS staff if you download and complete this spreadsheet.  Use complete sample names, not chemical symbols, when identifying samples.

Within this document, be sure to include:
  • Number of similar samples
  • Chemical Name (not chemical symbol!)
  • Form or phase (i.e. powder, solid, liquid, etc.)
  • Amount of material within each container
  • Value (if any)
  • Container Type (i.e. Kapton capillary, plastic vial, etc.)
